Position the foam strap behind the patient’s neck and route both ends through the tracheostomy tube neck flanges. Adjust the length by trimming or overlapping the hook-and-loop tabs so two fingers fit comfortably between the strap and the neck. Inspect the strap and skin daily and replace whenever soiled, frayed, or non-adhering. Always change the holder with assistance to avoid accidental tube dislodgement.
